I have a late 2013 MBP, with a 1TB Drive and 16GB Ram and GEForce Graphics. Of late the fans are running constantly. I’ve cleaned the fans and replace the thermal paste

Update (05/27/2020) @danj When I ran Diagnostics I got an error VHD002 Storage Error. I use a samsung 970 M2 (Nov 2018) with an adapter Also this is my kernel_task when I startup
Clearly you have a sensor which is telling SMC the system needs to be in Safe Mode. Besides the thermal sensors there are a few current & voltage sensors which I’m suspecting one is firing off for some reason. Did you have a liquid spill at some point? Did you clean your system using a spray cleaner directly on your systems display? I would run the onboard diagnostics. Restart your system and press the D key, did you get any errors? If you get an EFI error you’ll need to buy the full version of TG Pro to get the diagnostics it offers. Reference: Mac startup key combinations
